Look for a firm or rubbery mound beneath the nipple, nipple puffiness, symmetry or asymmetry between sides, and possible sensitivity. Gynecomastia in puberty is common and can improve within 6 to 18 months; persistent changes, rapid growth, pain, or discharge warrant medical evaluation.
